    TINGKAT PENGENDALIAN DIABETES MELITUS TIPE 2 BERDASARKAN HBA1C DI POLIKLINIK METABOLIK ENDOKRIN RSUD ARIFIN ACHMAD PEKANBARU

    Diabetes mellitus (DM) is a group of metabolic diseases, the characteristic is hyperglycemia that causes by abnormality of insulin secretion, insulin action or both of it. Unoptimal controlling of DM may causes metabolic disorders and cronic vascular disorders. The aim of this study was to know the Level Controlling of Type 2 Diabetes Mellitus Base on HbA1c in Department of Metabolic Endocrinology Arifin Achmad General Hospital Pekanbaru. The type of research was cross sectional descriptive. The samples are patients with type 2 DM according to inclusion criteria as 38 respondents. In this study, Respondents took HbA1c examination with HPLC method to evaluate the level of controlling DM. In this study it was found that the level controlling of DM type 2 of 36 respondents were in poor and moderate control criteria with the percentage of poor control was 63% and percentage of moderate control was 32%. Respondents who the level controlling of DM type 2 in good control criteria only 2 patients with percentage as 5%. From this study, it can be concluded that the level controlling of type 2 DM base on HbA1c in metabolic endocrinology Arifin Achmad General Hospital Pekanbaru is poor and moderate.

    GAMBARAN PEMERIKSAAN SITOLOGI SERVIKS WANITA PEKERJA SEKSUAL TIDAK LANGSUNG PADA HOTSPOT JALAN TUANKU TAMBUSAI KECAMATAN SUKAJADI PEKANBARU

    Cervical cancer is the third most frequent cancer in women in Indonesia. Indirect female sex worker are at increased risk for hence cervical cancer . This research was carried out at hotsspot Jalan TuankuTambusai, Kecamatan Sukajadi Pekanbaru aimed to describe the characteristics of respondent and the cervical smear result. The result showed from 31 indirect female sex worker , 3,2% were diagnose with squamous cell carcinoma (SCC), 19,4% with high grade squamous intraephitelial lesion (HSIL), 16,1% with low grade squamous intraephitelial lesion (LSIL), 6,5% with atypical squamous cell undetermined significance (ASCUS), 38,7% with negative for intraepithelial lesion or malignancy (Bacterial vaginosis, Trichomonas vaginalis, Candidiasis presents) and 16,1% were normal result.. The conclusion of the study that abnormal smears of indirect female sex worker at hotspot Jalan Tuanku Tambusai Kecamatan Sukajadi Pekanbaru and high risk of cervical smear should be referred for further follow up.

    PROFIL PASIEN ULKUS DIABETIKUM YANG DIRAWAT DI RSUD ARIFIN ACHMAD PROVINSI RIAU PERIODE 1 JANUARI-31 DESEMBER 2011

    Diabetes mellitus (DM) is a group of metabolic disease which characterized by hyperglycemia, caused by insulin secretion or insulin activity disorder or both. Uncontrolled DM could cause many complications, one of them is diabetic ulcer. The sample was 39 patients using total sampling. In conclusion,characteristics of respondents by sex diabetic ulcer patients obtained 56.42% were females, age group <60 years obtained 82%, and duration of suffering DM obtained 48.72% 0-5 years. Distribution of diabetic ulcers based on fasting blood sugar obtained 93.93% is bad.. Diabetic ulcers distribution based on the location of 47.91% was obtained at toes, feet and back legs respectively 16.67%, 10.41% ,other areas and heel respectively 8.33%. Distribution of diabetic ulcer based on Wagner criteria was obtained 33.3% Grade 4, Grade 3 obtained 25.6%, 23.1% obtained Grade 5, Grade 2 obtained 10.3%, 7.7% obtained Grade 1 and Grade 6 obtained 0%. Distribution of diabetic ulcer based on length of stay obtained 63.15% were> 7 days, 3-7 days obtained 28.94%, and <3 days gained 7.89%. Distribution of diabetic ulcer based on therapy obtained 89.7% were non-surgical, and surgical therapy obtained 10.3%.

    GAMBARAN INDEKS BARTHEL PADA PASIEN STROKE DENGAN LEUKOSITOSIS DI RSUD ARIFIN ACHMAD PROVINSI

    The Barthel Index currently being used by general practices, neurologists, and other medical division to assess progress and prospect of rehabilitation for patients with disability. Stroke patients especially with apparent neurological deficit post stroke were taken to rehabilitation phase supervised by medical rehab practice as soon as they declared stable by authorized doctor. The definition of stable itself are vary, from appropriate scores of Glasgow Coma Scale (GCS), cooperativeness, or sometimes by clinical judgement by authorized doctor. The clinical judgement includes consideration of the general state of patients, like, as in this research focusing, inflamation. Inflamation as a natural response for threat inside the human body e.g bacteria & viral infection, foreign bodies, etc. often occured in stroke patients, characterized by high body temperatures and elevated white blood cell/ leukocyte count, defined as leukocytosis. Some study has been assigned to analyzed that state, since stroke patients with leukocytosis at onset often had a worse prognosis than others; mostly death. It has been suspected that leukocyte activities as phagocyte are responsible to broke tissues around them, as in stroke particularly hemoragic stroke the blood clot that sits around brain tissue worsening the injury; evident by CT Scan in some study that lession volume are wider in stroke patients with leukocytosis. The purpose of this research is to measures whether leukocytosis did worsening the brain injury suffered by all stroke survivors thus resulting in poorer neurological deficit, affecting their activity of daily living (ADL). ADL in this research measured by Barthel Index, consisting of ten simple tasks that represents basic activity of daily living e.g independency of bathing, grooming, feeding and other motoric activity. The results are from 35 qualified patients whose dominantly characterized by men, aged above 50 years old and leukocytes above 10.000/uL, 13 patients (37,15 %) were in total dependency (Barthel index 0-20), 8 (22,86 %) were in severe dependency (Barthel index 25-40), 4 (11,42 %) in mild dependency (Barthel index 45-55), 9 (25,71 %) in light dependency (Barthel index 60-95) and only 1 patient declared independent. Conclusion more than half of subjects are categorized as poor impairment in terms of ADL and only one survivor are declared independent after hospitalized.     PERBANDINGAN JUMLAH KASUS POSTDURAL PUNCTURE HEADACHE PADA PASIEN PASCA SECTIO CAESAREA DENGAN ANESTESI SPINAL MENGGUNAKAN JARUM TIPE QUINCKE 25G DAN 26G DI RSUD ARIFIN ACHMAD

    Postdural Puncture Headache (PDPH) is an iatrogenic complication from spinal anesthesia caused by puncture or laceration from duramater which leads to leakage of cerebrospinal liquor (LCS). Many factors to influence the incidence of PDPH one of which is the size of spinal needle. The purpose of this research to know comparison the incidence of PDPH between Quincke 25G and 26G in patients with post sectio caesarea. In this cohort analytic study, 68 consecutive patients with post sectio caesarea under spinal anaesthesia accordance to the inclusive and exlusive criteria were included. Incidence of PDPH in the Quincke 25G needle is 4 repondents (11,8%) and the 26G Quincke is 2 respondents (5.9%). With the most common sites of pain on Frontooccipital, and severity were assessed using the Visual Analogue Scale (VAS). The results of this research showed that no significant relationship between the size of the spinal needle with the incidence of PDPH in sectio caesarea patient at the Arifin Achmad Hospital.

    Cervical cancer is the third most frequent cancer in women in Indonesia. Indirect female sex worker are in high risk for having cervical cancer. World Health Organization had recommended an alternative approach for developing countries with down staging concept of cervical malignancy with visual inspection with acetic acid (VIA) to detect cervical precancerous lesion. This research was carried out at hotspot Tuanku Tambusai Street, Pekanbaru aimed to describe the characteritics of respondent and to detect displastic cervical epithelial cells with VIA. The result showed from 31 indirect female sex worker, 52 % were diagnosed with positive acetowhite appereance (positive VIA). Conclusion of the study that displastic cervical epithelial cells of indirect female sex worker who has high risk for having cervical cancer should be reffered for further follow up.

    The purpose of the judical process in the Medicine Forensic Science was to diagnose on a drowning case. One of the process was the diatom examination. The diatom examination could be done on the sample of lungs tissue, kidney, hepar, small intestine, duodenum and bone marrow. The function of this diatom examination was to confirm whether the victim’s death because of he drowning or other reasons and to identify the diatom within the dead body then compared to the diatom in the territorial waters prior to confirm the location where the accident occured. This reseach was held in March 2013 in Bangko river Rokan hilir district. Based on this reseach there were four types of diatom found out namely Aulacoseira sp, Diatoma sp, Nitzcshia sp, and Isthmia sp. The lowest abundance was in the station I where one type of diatom where Aulacoseira sp (720 cells/L) found, while the highest abundance was found in the station II where Aulacoseira sp, diatoma sp, and Nitzschia sp (1520 cells/L) included within. In the station III was found the type of diatom and abundance (720 cells/L) where Aulacoseira sp, diatoma sp, and Isthmia sp were included within it.

    Diatomea investigation in the body of drowning victims is one of important investigation to determination the drowning victims location. Diatomea is sould in the lung or another organ from the body of victims as prove of drowning intra-vitality it means the victims are still alive when they are in the water and they are dead because of it. Diatomea species determining can be concluded as a supporting diagnose to identify the location of drowning victims by comparing the diatomea in the victims body with diatomea species in the water this research is a descriptive research which is purposed for recognizing the species and the diatomea abundant in the Rokan’s River Estuary of Bangko Sub-district and Batu Hampar Sub-district Rokan Hilir Regency this research sample is taken from five research of stasions as purposive sampling. This research was done in April and May 2013 at Rokan’s River Estuary in Sub-district Bangko And Subdistrict Batu Hampar Rokan Hilir Regency. Diatomea investigation was done by using Binocular microscope. This determining was done by using Masaharu and Yunfang atlas the abundant was searched by counted and numbered by using American Public Health Associations (APHA) formula. Diatomea was founded eight species they are: Aulocoseira sp, Diatoma sp, Nitzchia sp, Isthmia sp, Melosira sp, Navicula sp, Skeletonema sp, Bitdulphia sp. The abundant of diatomea is founded 11152,2 sel/L when the water comes up and I found 9132,6 sel/L when the water come down with the Diatoma sp as the most dominant diatomea as a mount as 4986,4 sel/L when the water comes up and with diatoma sp as the most dominant Diatomea as amount as 3799,8 sel/L.

    GAMBARAN PELAYANAN KESEHATAN BAGI NARAPIDANA ANAK DI POLIKLINIK LEMBAGA PEMASYARAKATAN KLAS II B PEKANBARU

    Juvenile delinquents as vulnerable groups need special attention in obtaining their optimal health. Inside the correctional institution, the realm of health handling cannot be disregarded, in order to keep good physical and mental condition among the juvenile delinquents. The aim of this research was to observe the description of health care for juvenile delinquent in polyclinic of correctional institution class II B Pekanbaru. This research used descriptive quantitative approach through inmate perception questionnaires about health care in polyclinic and qualitative approach through SWOT analysis on the health stakeholders. The results of this research described that the five dimensions of service quality were good enough, which were only 3 (three) of 16 (sixteen) items stated unsatisfied by the inmates. There were the availability of brochures, pictures or pamphlets that explain dangers of diseases, regular health checks, and also the alertness of officers to came to the blocks if there were sick inmates. In SWOT analysis, early screening on every new entry inmates was a strength, but the number of health personnel and facilities were remained as weaknesses. Health counseling for inmates was an opportunity while overcapacity was still a threat in polyclinic health care.

    ANGKA KETAHANAN HIDUP SATU TAHUN PENDERITA KANKER PARU DI RUANG RAWAT INAP PARU RSUD ARIFIN ACHMAD PEKANBARU PERIODE MARET 2010 – MARET 2011

    Primary lung cancer is major public health problem and the commonest fatal malignancy in the developed world. Survival of lung cancer is still poor because the development of clinically silent and undetected metastases in the majority of patients. Based on the research taking secondary data from the medical records obtained 32 patients with lung cancer. Largest patients were age group 45 – 64 years that was 26 cases (81,85). 27 people (84,4%) of the lung cancer patients were males and 5 patients (15,6%) females. The most main symtomps was dyspneu 13 cases (40,6%). It was found that mostly the cases are non small cell lung cancer. Most of the patient came to the Department of Respiratory in late stadium (75%). Largest complicated were metastases and one year survival of lung cancer was 11 cases (34,4%)

    PEMERIKSAAN BAKTERI AEROB PATOGEN SALURAN PENCERNAAN PADA USAP DUBUR PENJAMAH MAKANAN DI INSTALASI GIZI RS X

    Food handlers are one of the factors that lead to food contamination due to direct contact of the preparation, cleaning, processing, transporting and serving food. Food handlers carriers (the carrier of pathogenic bacteria) could potentially transmit to public community and patients in the hospital. The carrier is a source of contamination which is more important than case with clinical symptoms that carrier detection is important as a preventive measure and control the transmission of disease especially food handlers. This research method was descriptive laboratory to identify aerobic bacteria of enteric pathogens among rectal swab food handlers at Nutrition Installation of X Hospital. Data was collected by identifying of bacteria Salmonella sp, Shigella sp and Vibrio Cholerae on feces specimens was taken through rectal swab food handlers class II and III. The results of rectal swab examination obtained 7 people were positive (33.3%) containing aerobic bacteria of enteric pathogens. Out of 7 people, 5 people were positive Salmonella sp (23.8%) and 2 people were positive Shigella sp (9.5%). This research showed that the health status and qualified personnel of food handlers were not in accordance with Permenkes RI No.1096/MENKES/PER/VI/2011.

    Diatoms examination in the body of drowning death was an important procedure in determination of death location. The presence of diatoms found in the lung and the other organs were the intravital sign, proving the victim were alive on the water. Determination of diatoms species could be used as a supportive diagnostic in identifying drowning site by comparing diatoms between the body and water. The aim of the present research was to find out the species and abundance of diatoms in Kuantan river especially at Taluk Kuantan area. Samples were taken from three stations which decided purposively in May 2013. The results showed that 23 species diatoms were found, namely: Isthmia sp, Aulacoseira sp, Diatoma sp, Melosira sp, Eunotia sp, Tubellaria sp, Gyrosigma sp, Achnantes sp, Fragillaria sp, Eucampia sp, Pleurosira sp, Cyclotella sp, Skeletonema sp, Rhizosolenia sp, Stephanodiscus sp, Streptotheca sp, Tintinnopsis sp, Triceratium sp, Dactyliosden sp, Chaetoceros sp, Thalassionema sp, Meridion sp, dan Eucyridium sp. Total abundance of diatoms account were 1442 cells/L, with the Diatoma sp were the dominant diatoms in the water which abundance of 736 cells/L.

    Learning achievement and level of study skill have variations. One of factors that affect learning achievement is study skill. Study skill is a technique with strategy domination in improving understanding of learning in obtaining, maintaining, also to get and solve problem become more effective and productive. The purpose of this study was to determine correlation of level of study skill with learning achievement of Medical Faculty students of Riau University batch 2010. This study design was analytic with cross sectional approach. The subject of this study was Medical Faculty students of Riau University batch 2010. This study sample was taken by total sampling technique. This study used questionnaire of study skill to measure level of study skill and Study Results Card (SRC) to measure learning achievement. Statistical test used for correlation of personality type with learning achievement was Spearman test. This study results showed there was not significant correlation between level of study skill and learning achievement (p=0,307). The conclusion of this study was most of students’ level of study skill is skillful and most of students (Grade Point Average) GPA category were very satisfying.

    Based on data from the Cancer Registration Center of Indonesian Phatologist Doctors Association, breast cancer is the highest incidence of cancer in women with a percentage of 15.67%. Immunohistochemistry test is important in breast cancer as a predictive and prognostic factor. This test also becomes the basis for the classification of molecular subtypes of breast cancer. The purpose of this study was to determine the immunohistochemical profile in patients with breast cancer at the Arifin Achmad Hospital. It was a descriptive retrospective study using secondary data of breast cancer patients from Arifin Achmad Hospital in January 2010 - December 2011 period. Results obtained were 58 cases. Most of the breast cancer patients were in the age range of 40-49 years (41.38%). The hormonal status test results was mostly found in category of double negative or ER (-) and PR (-) (43.10%). HER-2 test results showed that the majority of patients had HER-2 negative (72.41%). Histopathological gradation was in grade III (50%). The most often stage found at stage III (46,6%). Dominant molecular classification of breast cancer was luminal A (43.10%%).

    GAMBARAN KEJADIAN KAKU LEHER PADA PASIEN PASCA SEKSIO SESAREA DENGAN ANASTESI SPINAL DI RSUD ARIFIN ACHMAD PROVINSI RIAU

    Existence of neck stiffness after spinal anesthesia for cesarean section had been reported. Neck stiffness after spinal anesthesia can reduce patients’ quality of life although never reported causing death. This study aimed to explore and describe the demographic data, needle types and sizes, onset, and the pain severity in patients whom the neck stiffness occurred. Total respondents of this study are 46 patients that taken by using consecutive admission method. The type and size of the needle to be used was assigned in a random manner. During the postoperative period, patients were visited daily and asked about the presence, onset, and the pain severity of neck stiffness. The overall incidence of neck stiffness after spinal anesthesia was 10,9% (n=5), two of nine in 25-gauge Quinckie (22,2%), two of sixteen in 26-gauge Quinckie (12,5%), and one of twenty in 27-gauge Quinckie (5%). The onset of neck stiffness is between 24-48 hours postoperative. One patient classified as mild pain and four patients as moderate pain. All of the neck stiffness resolved within 24 hours with conservative management. The authors conclude that incidence of neck stiffness after spinal anesthesia using 25-gauge Quinckie is higher than 26-gauge Quinckie and 27-gauge Quinckie.

    There are several factors that affect learning achievement, includingfactorsfrom wirhin a person (internal) and outside factors (external). The internal factors are physical health, intelligence, motivation, interest and personality. Personality is characteristic of a person in feeling, thinking, behave and adapt itself to its environment.The purpose of this study was to determine correlation of personality type with learning achievement of Medical Faculty students of Riau University batch 2010. This study design was analytic with cross sectional approach.The subject of this study was Medical Faculty students of Riau University batch 2010. This study sample was taken by total sampling technique. This study used questionnaire of personality type to measure personality type and SRC to measure learning achievement. Statistical test that used for correlation of personality type with learning achievement was lamda test. This study results showed that there was significant correlation between personality type and learning achievement with strength of correlation was very weak (r=0,000; p=0,000). The conclusion of this study was most of students had type A personality and most of students GPA category were very satisfying.

    Complementary feeding practices are often inappropriate, for example early feeding practices, no variation and inadequate of frequency of feeding. Therefore, the children experience undernutrition. Inappropriate complementary feeding practices are caused by low of knowledge and attitude of mothers. The purposes of the study were to describe mothers knowledge and attitude about complementary feeding practices and nutritional status of infants 6-12 months old. This study was conducted in 21 Posyandu (Health and Nutrition Integrated Service Center) that have taken place in Puskesmas Rawat Inap (Hospitality Community Health Care) Simpang Tiga working area, Kecamatan Bukit Raya, Pekanbaru. This descriptive designed study was conducted with mothers and infants aged 6-12 months as respondents. 75 infants were included by using propotional random sampling method. The data were taken by using interviews and questionnaries of mothers and measurements of weight for age (W/A) of infants. The result showed, mothers have ‘enough’ knowledge 53,3%, mothers have ‘positive’ attitude 98,7%, and 88,0% nutritional status of infants aged 6-12 months were good.

    The problem of narcotics, psychotropic and other addictive substances (drugs) has become global problem. The effort to prevent that abuse keep on promote, one of them by improving knowledge and attitudes through counseling. The aim of this study was to determine the influence of counseling on knowledge and attitude of students in Handayani Senior High School of Pekanbaru. This study was pre experiment design with one group pretest-posttest. The samples were 103 students. The instrument of this study were questionnaire which was filled twice, before and after counseling. Analysis of the results were carried out using hypothesis test of pair categories variable. The results showed knowledge of students before counseling were good (42,7%), moderate (49,5%) and low (7,8%). After counseling, the knowledge of students became good (93,2%), moderate (5,8%), and low (1,0%). The attitude of students before counseling were positive category (90,3%) and negative (9,7%). After counseling, the attitude of students became positive (98,1%) and negative (1,9%). The results showed that there was difference of knowledge after counseling with the value of p=0,000 and there was difference of attitude after counseling with the value of p=0,008. The conclusion was the counseling influences on improving students knowledge and attitude about drugs in Handayani Senior High School of Pekanbaru

    Painful menses or dysmenorrhea is a common complaint among adolescent womans, dysmenorrhea is characterized by lower abdominal pain that occurs during menstruation, but may start two or more days before menstruation. Sometimes, pain is associated with headache, nausea, vomiting, gastrointestinal symptoms, and others. Dysmenorrhea known as primary dysmenorrhea and secondary dysmenorrhea, Primary dysmenorrhea which is defined as painful menses in women with normal pelvic anatomy and secondary dysmenorrhea is defined as painful menses in women with pelvic organ pathology and the most common cause of the secondary dysmenorrhea is endometriosis. The purpose of this research to know the overview of dysmenorrhea with endometriosis among female students in Medical Faculty of Riau University. This Research was a descriptive study and already done on March until April 2013. The result of this research are from 215 female medical students there are 87,44% of them experiencing dysmenorrhea,50 female students who agree to do the ultrasonography examination and 36% of them were found endometriosis. The most dysmenorrhea onset is seldom occurs in 55,56%. Most common time of dysmenorrhea is during a period of menstruating occurs in 44,44 %. Most typical pain in dysmenorrhea is progressively alleviate occurs in 94,44%. Family history with dysmenorrhea found 50% cases. Usage of analgetics found in 27,78% cases and most level pain in dysmenorrhea is 1 and 2 degree with each percentage is 44,44%.
