Diabetes mellitus (DM) is a group of metabolic diseases, the characteristic is hyperglycemia that causes by abnormality of insulin secretion, insulin action or both of it. Unoptimal controlling of DM may causes metabolic disorders and cronic vascular disorders. The aim of this study was to know the Level Controlling of Type 2 Diabetes Mellitus Base on HbA1c in Department of Metabolic Endocrinology Arifin Achmad General Hospital Pekanbaru. The type of research was cross sectional descriptive. The samples are patients with type 2 DM according to inclusion criteria as 38 respondents. In this study, Respondents took HbA1c examination with HPLC method to evaluate the level of controlling DM. In this study it was found that the level controlling of DM type 2 of 36 respondents were in poor and moderate control criteria with the percentage of poor control was 63% and percentage of moderate control was 32%. Respondents who the level controlling of DM type 2 in good control criteria only 2 patients with percentage as 5%. From this study, it can be concluded that the level controlling of type 2 DM base on HbA1c in metabolic endocrinology Arifin Achmad General Hospital Pekanbaru is poor and moderate.

Cervical cancer is the third most frequent cancer in women in Indonesia.
Indirect female sex worker are at increased risk for hence cervical cancer . This
research was carried out at hotsspot Jalan TuankuTambusai, Kecamatan
Sukajadi Pekanbaru aimed to describe the characteristics of respondent and the
cervical smear result. The result showed from 31 indirect female sex worker ,
3,2% were diagnose with squamous cell carcinoma (SCC), 19,4% with high grade
squamous intraephitelial lesion (HSIL), 16,1% with low grade squamous
intraephitelial lesion (LSIL), 6,5% with atypical squamous cell undetermined
significance (ASCUS), 38,7% with negative for intraepithelial lesion or
malignancy (Bacterial vaginosis, Trichomonas vaginalis, Candidiasis presents)
and 16,1% were normal result.. The conclusion of the study that abnormal smears
of indirect female sex worker at hotspot Jalan Tuanku Tambusai Kecamatan
Sukajadi Pekanbaru and high risk of cervical smear should be referred for further
follow up.

Diabetes mellitus (DM) is a group of metabolic disease which characterized by
hyperglycemia, caused by insulin secretion or insulin activity disorder or both.
Uncontrolled DM could cause many complications, one of them is diabetic ulcer. The
sample was 39 patients using total sampling. In conclusion,characteristics of respondents
by sex diabetic ulcer patients obtained 56.42% were females, age group <60 years
obtained 82%, and duration of suffering DM obtained 48.72% 0-5 years. Distribution of
diabetic ulcers based on fasting blood sugar obtained 93.93% is bad.. Diabetic ulcers
distribution based on the location of 47.91% was obtained at toes, feet and back legs
respectively 16.67%, 10.41% ,other areas and heel respectively 8.33%. Distribution of
diabetic ulcer based on Wagner criteria was obtained 33.3% Grade 4, Grade 3 obtained
25.6%, 23.1% obtained Grade 5, Grade 2 obtained 10.3%, 7.7% obtained Grade 1 and
Grade 6 obtained 0%. Distribution of diabetic ulcer based on length of stay obtained
63.15% were> 7 days, 3-7 days obtained 28.94%, and <3 days gained 7.89%. Distribution
of diabetic ulcer based on therapy obtained 89.7% were non-surgical, and surgical therapy
obtained 10.3%.

GAMBARAN INDEKS BARTHEL PADA PASIEN STROKE DENGAN LEUKOSITOSIS DI RSUD ARIFIN ACHMAD PROVINSI

The Barthel Index currently being used by general practices, neurologists,
and other medical division to assess progress and prospect of rehabilitation for
patients with disability. Stroke patients especially with apparent neurological
deficit post stroke were taken to rehabilitation phase supervised by medical rehab
practice as soon as they declared stable by authorized doctor. The definition of
stable itself are vary, from appropriate scores of Glasgow Coma Scale (GCS), cooperativeness,
or sometimes by clinical judgement by authorized doctor. The
clinical judgement includes consideration of the general state of patients, like, as
in this research focusing, inflamation. Inflamation as a natural response for
threat inside the human body e.g bacteria & viral infection, foreign bodies, etc.
often occured in stroke patients, characterized by high body temperatures and
elevated white blood cell/ leukocyte count, defined as leukocytosis. Some study
has been assigned to analyzed that state, since stroke patients with leukocytosis at
onset often had a worse prognosis than others; mostly death. It has been
suspected that leukocyte activities as phagocyte are responsible to broke tissues
around them, as in stroke particularly hemoragic stroke the blood clot that sits
around brain tissue worsening the injury; evident by CT Scan in some study that
lession volume are wider in stroke patients with leukocytosis. The purpose of this
research is to measures whether leukocytosis did worsening the brain injury
suffered by all stroke survivors thus resulting in poorer neurological deficit,
affecting their activity of daily living (ADL). ADL in this research measured by
Barthel Index, consisting of ten simple tasks that represents basic activity of daily
living e.g independency of bathing, grooming, feeding and other motoric activity.
The results are from 35 qualified patients whose dominantly characterized by
men, aged above 50 years old and leukocytes above 10.000/uL, 13 patients (37,15
%) were in total dependency (Barthel index 0-20), 8 (22,86 %) were in severe
dependency (Barthel index 25-40), 4 (11,42 %) in mild dependency (Barthel index
45-55), 9 (25,71 %) in light dependency (Barthel index 60-95) and only 1 patient
declared independent. Conclusion more than half of subjects are categorized as
poor impairment in terms of ADL and only one survivor are declared independent
after hospitalized.
